1. Airway: Avoid hypoxia and acidosis; ventilate early.
2. Seizure control: Benzodiazepines preferred. Avoid large doses of propofol.
3. ACLS: Use ≤1 mcg/kg epinephrine. Avoid lidocaine, β-blockers, calcium channel blockers, vasopressin.
4. Lipid Rescue: Bolus: 1.5 mL/kg of 20% lipid emulsion over 2–3 min Repeat every 3–5 min if needed (max 3 mL/kg bolus) Infusion: 0.25–0.5 mL/kg/min for ≥10 min post-stability Max total dose: 12 mL/kg in 30 min
